Il film è ambientato a Londra nella seconda metà dell’Ottocento e segue la vera storia di Joseph Merrick (John Hurt), un giovane deforme costretto a esibirsi insieme ad altri fenomeni da baraccone in uno show di strada gestito da un uomo malvagio, Bytes (Freddie Jones).
Durante uno di questi spettacoli in giro per la città, il protagonista, soprannominato Uomo elefante a causa della Sindrome di Proteo che gli deturpa il volto, viene notato da un dottore, Frederick Treves (Anthony Hopkins), che lo porta con sé in ospedale per mostrarlo ai colleghi. Per non spaventare le infermiere, Merrick viene messo in isolamento sotto le cure della caposala, Madre Shead (Wendy Hiller), contro il parere del Direttore Carr Gomm (John Gielgud), che non vuole tenere in clinica persone incurabili.
Ma l’Uomo elefante si rivela col tempo una persona sensibile e intelligente, qualità che aveva tenuto nascoste per evitare di essere maltrattato dal suo aguzzino Bytes.
Nel frattempo la storia di Merrick arriva fino alla corte della regina Vittoria, che decide di istituire un fondo monetario per pagargli le cure. Nonostante l’uomo riceva le attenzioni di moltissime persone, tra cui una famosa attrice teatrale, il dottor Treves e Madre Shead si chiedono se non lo stiano trattando ancora come un fenomeno da baraccone.
Quando finalmente arriva la notizia che i fondi per il mantenimento permanente di Merrick sono stati finanziati, l’Uomo elefante viene rapito dal malvagio Bytes, cambiando completamente le sorti del suo destino…
